Office Policies

For All Visits
Please always bring:
1. Photo ID
2. Insurance card
3. Updated medication list
4. Complete online check in process via the link provided in your email/patient portal or cell phone
5. If you have recent bloodwork from an outside facility


Office Policies
1. All co-pays, deductibles and balances are due at the time of your visit.
2. If you are not insured, we require payment at the time of service.
3. We accept all major credit cards (preferred) and checks, and cash.

Cancellation Policy:
Please notify us at least 48 hours prior to your appointment to cancel or change your appointment. A no-show or late cancellation will result in a $50 fee.

Patient Responsibility:
If your insurance requires a referral, it will be your responsibility to obtain the insurance referral.
If you are unsure if a referral is necessary, please check with your insurance company. Otherwise, you will be considered a self-payer.
